- Notes for filling out the application for investigation:
- Both the person involved and the whistleblower must sign their names.
- Whether typed or handwritten, the application form must be signed in person.
- Faculty and staff should fill out the investigation form under the Gender Equality Work Act (responsible unit: Personnel Office).
- Students should fill out the investigation form under the Gender Equality Education Act (responsible unit: Gender Equality Education Committee).
